By completing this form, your tax will be reclaimed on any eligible donations or membership subscriptions you have made (or will make) to the QRA until further notice. You confirm that you pay an amount of UK income or capital gains tax at least equal to the amount the QRA will reclaim from the subscription.

Please notify the QRA in future if you:
  1. Want to cancel this declaration
  2. Change your name or home address
  3. No longer pay sufficient tax on your income
If you pay Income Tax at a higher or additional rate and want to receive the additional tax relief, you must include all your Gift Aid on your Self-Assessment tax return or ask HMRC to adjust your tax code.
